Family Law Intake Example

We have recently worked with a family lawyer to create a Cognito Forms intake process.  The process has the following key features:

  1. The form initially only captures some very basic information that is used to assess if the client is a good fit for the firm.  If they are not, the form directs them to some other lawyers that may be a better fit.  If they are a good fit, the form opens up further screens where the client can enter the required amount of information for the initial consultation. 
  2. Following the initial consultation, if the matter will proceed, the firm can change the status of the form which opens up further details.  Effectively the client is now asked for all of the information on their assets and liabilities that the firm would need to draft the Affidavit of Assets and Liabilities in Schedule 8 of the High Court Rules.  This includes uploading all the key documents required.  For the firm, this is much better than receiving information over multiple emails. 
  3. The client has the ability to save and resume, so they can gather the information in their own time.  The firm can see the status at any time. 
  4. We created an automation action in the form for the firm which will automatically download (using a Zapier workflow) all attachments and save them (with a set naming convention) into SharePoint. 
  5. In a later phase of the project, we will be able to create documents for the firm relevant to the matter, such as separation agreements, affidavits or other court documents.
